Soulsational Festival Music and Poet Performance Application

After submitting form below please submit one band press photo, band plot map, and an example original song mp3 emailed to [email protected] . All final main stage performances are chosen by a voting as a Soulsational committee. The musical feel of the festival is folk, blues, soul and anything with a Jersey shore feel. Music must be 75% original. We requests all musical and poetic sets performed to maintain a positive demeanor, this is a free family event. This event is focused around living a POSITIVE LIFESTYLE.

All Poets must also fill out this form, skipping what doesn't pertain to them. They MUST submit Poetry Pieces they would like to use at Soulsational for prior approval. We do not allow material that is not family friendly. Please be mindful there are children everywhere.

Could you, in as much detail as possible, tell me what your performance needs will be for the day’s event. Any information you give me, no matter how minor it may seem, will help us be prepared for you. We do not want to disappoint you day of show by not having some essential piece of equipment that you expected to be there and we did not have. Thank you MAIN STAGE Sound Engineer. (this does not apply to pavilion stage)

I agree by electronic signature to all of the following Soulsational musical performance details. Main Stage Compensation: Will be determined by The Soulsational Committee if budget allows. We a free family festival that relies on volunteers. Time slots being filled: 11:15am, 12 noon, 1:00pm, 2:00pm, 3:00pm, 4:00pm, 5:00pm, 6:00pm. This is subject to change slightly. Set performances are 30 minutes, 45 minutes, and 60 minutes. This is subject to change slightly. Soulsational will have a 10’x10’ tent next to the Main Stage with a table and chairs set up for bands to sell their merch during their performance and for 1 hour after. Submitting an application does NOT guarantee you a performance spot at Soulsational Music & Wellness Festival. At times this space will be shared. All bands and artists when selected will be notified upon committee approval. All items listed above are required before an application is complete. We reserve the right to request you do NOT perform within two weeks; and within a one hour radius of this event. Submission materials will not be returned. There is NO Performance application fee at this time. Acceptance ONLY notifications will be sent approximately April 10th for Main stage and May 1st for the pavilion stage and posted on website. Accepted bands/artists have one week upon notification of acceptance to reply back and confirm. If we receive no response, we hold the right to cancel acceptance and move forward with next highest voted to play. All performing bands and artists will be listed on our website no later than June 1 - 10th. Bands and artists should place a hold on the date of the festival for availability, as acceptance will require the band to perform on the last Saturday of July. Soulsational will notify all bands as soon as possible upon acceptance. This courtesy is appreciated. Declinations will not be sent. Bands understand that they will not be compensated through performance rights organizations or royalties for performance at this festival. Bands are to arrive no later than 60 - 90 minutes prior to performing time, which will be scheduled between 11am -6pm. The festival will supply the back-line: PA, sound equipment, monitors, microphones and a drum set to help ease the transition between bands. Band direction will be conducted by a stage manager which will have been assigned prior to event for the main stage. As we strive to create a festival of good will and integrity, we reserve the right to deny any applications at our discretion if they conflict with the overall vision of the Soulsational Festival. Soulsational with the help of the sound company Equipment will be providing a complete large scale concert sound system, with on stage monitors, wired and wireless microphones (handheld only), instrument microphones, microphone stands, direct input boxes, and cables. We are also providing a shared drum kit for the day. Monitors will be mixed from stage and front of house will be mixed approx 75 feet from the lip of the stage. This is a Rain or Shine Event. At the last minute on the day of the event some time adjustments may have to be made due time restraints. I have read and understood this Soulsational agreement: This is my Electronic Signature.

Electronic Signature Needed: As a condition of participating as a vendor, exhibitor, band member, participant or volunteer at the Soulsational Music & Wellness Festival (the “Festival”), I, _____________________ acknowledge that I fully understand the risks inherent to participation in the Festival, and I expressly agree, on behalf of myself, my heirs, executors, administrators, successors, and/or assigns, to release and hold harmless Soulsational Music & Wellness Festival, Venue location, Alternative Health Solutions of NJ, Michelle Leonard and their respective subsidiaries, affiliates, agents, officers, directors, managers, volunteers and employees (each of the foregoing, collectively, the “Releases”) from and against, any and all damages, losses, expenses (including attorneys’ fees and costs) or injuries, and any and all other claims of any type whatsoever for any personal injury, death, disability, or damage to tangible or intangible property, or any other loss, damage, or injury of any nature whatsoever (collectively, “Claims”) relating to or sustained or caused during the Festival. Furthermore, I agree, on behalf of myself, my heirs, executors, administrators, successors and/or assigns, to indemnify and hold harmless the Releases from and against any and all Claims made by any third party arising out of my participation in the Festival. Such obligation shall not be construed to negate, abridge, or reduce other rights or obligations of indemnity which would otherwise exist as to any party or person described in this Release and Indemnity Agreement. I agree to keep confidential and not to disclose any non-public information, regarding the festivals or any of the Releases, disclosed to me during the course of my participation in the Festival. I give and grant to Soulsational Music & Wellness Festival, Alternative Health Solutions of NJ, Michelle Leonard and its officers, parent companies, employees, agents, representatives, affiliates, subsidiaries, consultants, independent contractors, successors, assigns, designees and licensees ("LIG"), the right to use and publish my name, nickname, voice, picture, portrait, likeness, characteristics, live or recorded performance, photograph, signature or facsimile thereof, and biographical materials (the “Identity”) as embodied in materials associated with Soulsational marketing, advertising, sales or promotional efforts (the “Materials”) in all forms of media now known or hereafter created, including without limitation, video, television, print, billboards, point-of-purchase materials, corporate communications, internal and external marketing pieces, and Internet in any and all territories throughout the world. In the event that any clause, term, or provision of this agreement shall be deemed or adjudicated void or invalid, it shall in no manner affect the validity or enforceability of the remaining clauses, terms, and provisions herein which shall remain in full force and effect. I hereby acknowledge that I have read the Release and Indemnity Agreement and am satisfied that it is fair and equitable. I shall not revoke this consent and approval. I have read and understand the Soulsational liability release and indemnity agreement: This is my Electronic Signature.

Electronic Signature Needed: I agree to be an active participant in marketing and awareness of the festival; especially my own set time. I agree to obtain marketing material to disperse on arrival. Each person is to be help spread awareness. I understand if for some reason I cancel my set without a doctor or hospital note after material has been printed I will be responsible for the full cost of the new marketing material to be printed.

I agree that if I am accepted, I am responsible to follow through with the class as advertised on the day of the event. If I cancel my space after the printed material is purchased by the festival, I am responsible to have that printed material reprinted at the printers fees. I agree that if I do not show up or cancel for any other reason before the event other than a documented extreme emergency, I will be held responsible for all advertising costs which displayed my name or band name. I have read and understand this Soulsational agreement: This is my Electronic Signature.
